Broadway Services, Inc., its agents, representatives, employees and third party vendors (collectively, “BSI”) is making available various forms of electronic communication to its employees. BSI intends to communicate with its employees electronically with respect to notifications about BSI benefits; Summary BSI Annual Reports; Summary BSI Benefits and Coverage; Notices that provide links to Income Tax forms such as W-2, W-4 and 1095 forms and such other forms and documents which BSI may elect to include from time to time (the “Documents”). I HEREBY CONSENT TO RECEIVE THE DOCUMENTS BY MEANS OF ELECTRONIC COMMUNICATION. I ACKNOWLEDGE THAT I HAVE READ AND FULLY UNDERSTAND THE RISKS, LIMITATIONS, CONDITIONS OF USE, AND INSTRUCTIONS FOR USE OF THE SELECTED ELECTRONIC COMMUNICATION FOR RECEIPT OF THE DOCUMENTS.

I understand and accept the risks herein to this consent form, associated with the use of the electronic communications with BSI. I consent to the conditions and will follow the instructions outlined herein, as well as any other conditions that BSI may impose on communications with its employees in receiving the Documents.

I acknowledge and understand that it is possible that electronic communications with BSI may not be encrypted. Despite this, I agree to communicate with BSI using these electronic means with a full understanding of the risk.

I may revoke my consent to use of electronic communications and may obtain a paper copy of any of Documents from BSI. I may contact BSI at 3709 E. Monument St. office or submit updated consent form on-line at https://www.broadwayservices.com/BSI2017/ElectronicCommForm in order to elect out of using electronic communications with BSI.

BSI will use reasonable means to protect the security and confidentiality of information sent and received using electronic communications. However, BSI cannot guarantee the security and confidentiality of electronic communications: • Despite reasonable efforts to protect the privacy and security of electronic communication, it is not possible to completely secure the information. • Electronic communications can introduce malware into a computer system, and potentially damage or disrupt the computer, networks, and security settings. • Electronic communications can be forwarded, intercepted, circulated, stored, or even changed without the Knowledge or permission of BSI. • Even after the sender and recipient have deleted copies of electronic communications, back-up copies may exist on a computer system. • Email, text messages, and instant messages can more easily be misdirected, resulting in increased risk of being received by unintended and unknown recipients. • Email, text messages, and instant messages can be easier to falsify than handwritten or signed hard copies. You may be charged a fee through your internet company for emails, text messages or instant messages.

It is your responsibility to provide BSI with true, accurate and complete contact information and BSI will not be held responsible for your failure to provide accurate contact information in order to use electronic communications. In order to receive electronic communications, you will need to have internet access (Microsoft Edge, Google Chrome or Apple Safari); an email address which allows you to send and receive emails; and either Microsoft Word or Adobe Acrobat Reader.
